What I meant is that it appears that way back in 1964, NASA thought one astronaut at a time would go out (the first one returning back before the second egressed. The hatch was also different (meant for CM docking) so we'll never know if it would have been workable or not.
As you said (and seen in the TV serie as well as mentioned in many books), those ideas evolved pretty much with time.
(My small regret was that NASA didn't keep the red helmets worn during Apollo 9! It would have been quite dashing:-)
