Re: altitude
It's mainly the Soyuz and Progress altitude capabilities that determined the ISS altitude (same as Mir). ISS being much larger (in the near future), it will be much harder to keep it at its altitude, but there are means like more frequent Progress boosts, Shuttle pushing the station and also the ATV use, later on.
