I've been struggling with that for months. You summed it up nicely with the description of the header being "carried" along--my big problem was to make the pages aware of where they belonged, so the headers would be right; and one technique I came up with is for a page to look at the last page, the one that referred it, and mimic its layout.
I'm working on that this week. With luck I can make it better soon.
