It's to replace the Soyuz vehicle that has a 6 months lifespan in space.
Budarin is not "ill"....He is not ready for a spacewalk, but i haven't heard of plans to replace him.
But that would be in a Shuttle flight and the next one is one of the last ones not involving either ISS or the HST.
