Tougher medical exams at NASA

Looks like Budarin (in space) and now Vinogradov and Poleshuk have not succeeded in medical exams. The latter two are grounded.
I remember Vinogradov together with Soloviov, saving Mir during the 1997 summer. They performed brilliantly, made a number of spacewalks.
And now, NASA with far less experience in long duration spaceflights, declares unfit cosmonauts with already months of experience in space and in spacewalks.
