Nice report, everything you ever wanted to know about tiles and then some...
Though the tiles do not appear to be the root cause (but there were clearly some thermal issues).
The report clearly states, the most common source for (tile damaging) debris comes from the insulation protecting the external tanks/boosters. Interesting, kind of shoots down the theory that the insulation could not have damaged the tiles.
