Membership may be required to view these files, but registration is not troublesome. The file is a powerpoint slide presentation, so powerpoint viewing software will be required. it's available free at the microsoft office website and from other sources.
IMPORTANT NOTE! The impact location noted on slide 2 is MY EDIT, not NASA's. That is my best guess as to where the foam debris hit the wing on ascent.
