Specifically, in the handling of the tile damage impact report. The turning down by NASA management of request to Mauii for on-orbit images is inexcusable - I am looking forward to Rocha et al's testimony here, as well as the Readdy/Ham/Dittemore e-mails.

Once again, depending on the initial damage state, there were techniques available for either modified re-entry, or on-orbit re-supply.

This is a $3.5 billion, 7 crew, and 2 recovery crew loss.
