Yes, the collision at Tenerife between the KLM and the Panam 747's...I believe in 1977.
Astronaut Wubbo Ockels (STS61A) provoked a collision on the ground, with an Airbus in a french airport. His argument was that the controler wasn't speaking a good enough english.
Well, since, Wubbo had lost all hopes of returning back to space.
