Leaping to conclusions
In the sad aftermath of the Columbia tragedy, I noticed two comments which I think are valuable, one from a member of our own HZ community:
"I can't rush to judgment on it," he cautioned. "A lot of things in this business that look like a smoking gun turn out not to be true."

"NASA is exactly right that they can't get locked into any one scenario so early in the investigation. Fixing the wrong problem won't do anything to improve safety."
