Probable IFF failure and a mid-air collision
Night helicopter operations over water are damned dangerous at any time. A mid-air collision or crashing into water is a tragically all too common event.
As for the Tornado, the most probable cause was an IFF failure. If I'm right, the Patriot crew saw an unidentified aircraft moving at high speed towards Kuwait and shot it down. The Tornado was returning from a bombing mission, so this one was combat related.
