I had it done to my son as an infant, and they did indeed use a locally injected dermal anesthetic, similar to what they'd give you for getting stitches. I noticed no particular discomfort on his part at the time (other than the shot), and the annoyance at being restrained from moving. I don't know what the traditions are for using anesthetics for religious circumcisions.
