The quote of Admiral Hyman Rickover says it well.
There needs to be an overriding policy that demands investigation, solution and resolve of any anomaly of any kind that would result in the alteration of a flight ready vehicle. This would mean that anything out of the ordinary would override any decision to continue the flight until the anomaly was investigated and or fixed without regard to launch delay, severity, cost and mission.
The Challenger never should have launched that day.
Columbia's wing should have been investigated with a solution not an assumption before the return flight.
No more management decisions like; it should be OK, just do it as standard policy.
"Failure is not an Option" needs a new direction.
