It's a browser compatibility issue
I see by the string at the bottom of your post that you, too, are a Firefox user. When I first open a TOC, the "earlier" and "reload" buttons, and the bottom frame they're in, appear momentarily until the TOC loads -- then they disappear.
You may also have noticed there's no HTML preview for such as we.
