I got a good one recently
I keep a MSN dial-up account that I pay for annually via debit card. Every few years the debit card expires and I have to update the exp date.
I got a very decent looking email supposedly from MSN requesting update info. The reply link was dead so I guess they were caught but if the reply site had been as convincing as the email, they might have gotten me with that one.
