I would guess that this statement:

...is BS since it is so incredibly hard to determine what/who started something like this. The one time I was in a crowd when something bad happened, no one could figure out what started the panic (and in my case the entire mob-full of witnesses survived).

Could have been a terrorist, I suppose. Strange shift in tactics, if so. Anyhow the terrorists are ultimately responsible since if people did not have a credible "fear of persons with explosive belts", then this panic would not have happened.
