No. There are acts of God. There is sabotage. There is negligence. There are random happenings that may occur but are accepted as being within design and error margins. There are may degrees of human error as well. Obviously, fundamental design flaws are much more serious, than procedural errors that may be easily corrected. Countless books are written categorizing error types and how systemically to eliminate or minimize them. To speciously reduce them all to "human error" is not very useful lol.
