Components disengaging from the shuttle as spare parts floating around the vacinity seems a bit odd to me. The rule of thumb in the aircraft industry is if a bolt or washer is dropped and can't be located immediately, that craft is grounded until every peice is secured and accounted for. How does quality control work at NASA?
