According to his website, Hawking's 24 hour nursing care is provided by "grants from several foundations," not to mention housing accommodations provided by the college. I have no idea how much of his care is paid for by the NHS, and how much by himself and other sources.

It goes without saying that the rich have few problems dealing with below-par health care systems in any country.
