We go into the fray. Instead of RU launching a nuke, North Korea does, on US, or Japan, or South Korea. On Putin's behest with plausible deniability. Or just because he's even crazier than Putin. Then the response would have to be against Pyongyang, not Moscow. Another front.
(edit: That's pretty much what Cuba was being set up for, IMO.)
