The problem is that the Shuttle tile material is too brittle for anything other than an adhesive approach. Even drilling a hole through one creates problems. There are metallic TPS systems that do indeed use fasteners. I've seen NASA prototypes, and a lot of work was done on metal TPS for the X-33.
