I think the problem in the Gulf War was a warhead timing error. Those Patriot missiles used an explosive warhead to destroy the target. However, when you consider that the incoming Scud was doing something like Mach 3 and so was the Patriot, well, a closing speed of Mach 6 doesn't leave much margin for error. A timing error of even one millisecond would be enough to miss destroying the warhead. Those Scuds had also been modified and frequently broke up on the downward flight (V-2s had the same problem). That made targeting much harder.
