1. No, Iraq never launched a satellite. You're probably thinking of Israel, who has launched a few satellites, the Ofeq series. Interestingly enough, their first few satellites were launched into energy intensive retrograde orbits because they didn't want to overfly hostile neighbors.
2-4. Like Tom pointed out, you don't need to launch a satellite into orbit in order to target another satellite or the ISS. Back in the 1980s, the US tested an air launched (from an F-15) ASAT that actually destroyed an old satellite on one test. The ASAT was built using a SRAM (short range attack missile) rocket as a booster and an earlier version of the miniture homing vehicle used on the ABM tests. The vehicle never entered orbit. It worked but was cancelled by Congress.
