...Seriously, backup chutes are one option. Our design had a blowout panel and bailout capacity if the chute screwed up. No piece of cake, but it was there. I remember reading about such a falling capsule bailout in an early high-altitude balloon flight. A real sphincter-clencher.
Standard Shuttle Clark flight suits include a parachute.
