General Dynamics had such a plan, involving the Shuttle, a Titan IV and an Upper G' Centaur stage.
I'm afraid we don't have the technology yet to keep a fully loaded Tug for more than a few hours in LEO, before it leaks enough LH2 to make the trip to the Moon impossible.
