Scott writes a bit about this parody (sort of like a Charlie Chaplin mute movie with a piano sound) in his latest book. It lasts about 15 minutes.

Leonow writes about the Soyuz 11 tragedy (it is known he should have been the commander of that flight). Before re-entry, he advised the crew to close the air vents betwen the landing and orbital modules and not to forget to re-open them after parachute deploy. According to the flight programme, the vents were supposed to close and then open automatically once the parachute had deployed. Leonov believed the vents might open prematurely.
That' what happened, unfortunately. It killed the crew. According to Leonov, it was human error.
