Was it breached purposefully? Yes! The entire levy system purposefully designed to fail if a Cat 4 or above storm hit!

So yes, it was purposefully breached! The funny thing is, the engineers who designed it probably didn't even know they were all a key part of the Bush family's Masterful plan.
