I had a discussion about Aldrin's hatch issue (in a french space forum) because Russel Schweickart who was the Apollo 9 LMP (right to Mc Divitt, the CDR) did a spacewalk from the LM (and not the CDR)...
So how come Rusty did it and Buzz had to wait for Neil to get out?
Perhaps..perhaps, it's because Rusty could get out with his head first, his back towards the LM porch (because of weightlessness) while Neil had to leave the LM with his feet first and the belly facing the porch.
