I hadn't made the connection with Schweickart egressing the LM. I won't spoil it for you, but Hansen does a thorough analysis of how Neil came to be the first out the door, and what he reveals, through statements from Slayton and Kraft, is a bit surprising. I think the question is finally answered in this book.
