A story I heard on NPR yesterday morning indicates that BP had postponed diagnostic and maintenance procedures on the pipeline for over a year (reason not specified, but I suspect it's the usual excuses: to save money and ensure uninterrupted supply of crude) and this may have contributed to the problem and its severity. Apparently, the maintenance involves shutting down and draining the pipeline and sending a robotic probe called a "bug" through it to check it out.
