Two things that could've happened.
The first is that their nukes are as malnourished as their population, and like their missiles, the test was a complete flop. The second thing is that all they wanted to test was the mechanics of their design. It may have had just enough fissionable material to test the configuration of the conventional explosives so they could see if the design was valid.
