It's important to separate the Third World situation from ours. The Third World situation is still bad, even if it's not as bad as painted (unless there are further "corrections" down the road).
As you point out, it became clear quite early on that in the U.S., unless you were born to an infected mother, or got unlucky with a transfusion, that you practically had to get AIDS on purpose. Between that, and much better treatments, it's not much of a political issue any more.
