Could it have been double-hulled?
With a much longer gash through the outer hull and the 6-inch hole through the inner hull?
That wouldn't explain the failure to shut water-tight compartments, or the failure of the pumps, I know. But may explain a somewhat slow reaction.
As you can tell I know next to nothing about ship construction.
