Fist Sized Hole
I suspect that someone saw a fist sized hole in a vsible part of the inner hull. There is likely to have been far more extensive but hidden damage.
Someone with more time on their hands could calculate the flooding rate by looking at time taken for a substantial list to develop, and I think it'll take more than a fist-sized hole to do that, watertight doors or not...
