IMHO, it is largely randomness.
Consider the randomness of human beings.
Each of us derives from the union of a single egg cell with a single sperm cell. Most women release only one egg per cycle, but men release hundreds of millions of sperm cells with each ejaculation. These sperm cells are so tiny that they are affected significantly by Brownian Motion. Hence, the very genetic complement of each and every one of us is randomly chosen from a huge population of alternatives.
Furthermore, there are countless unpredictable and uncontrollable random factors that affect our lives. Traffic accidents kill nearly 40,000 USA citizens each year and injure millions of others.
And so on.
