MAYBE...the lack of action is due to one wrong decision by an incident commander who was not even present at the scene, or perhaps it was caused by blind obedience to a playbook that left out one key observation. Remember, responders are paramilitary, they are trained to follow orders. Kids were calling 911 for help, but do the dispatchers communicate in real time with the on-scene personnel? I used to work in 911, (I edited the computer maps and databases that convert lat/lon positions to addresses), that's not how its set up.
There's no doubt someone is at fault for this disaster, even if it all turns out to be a procedural error, but I'm more inclined to think it was incompetence rather than downright malice. No one ever died because of my fuck-ups, but I know how easily they might have.
