Even though it's a horrific thought that Israel might be the target of a madman's bomb from Iran, somehow in my gut I know that Israel would retaliate with their full force and the rest of the world would sit back and let them duke it out. The moron in Iran would have to know that and so it would not only be his own suicide but suicide for his country as well.
You'd have to hope that there were intelligent people at the top that would stop him from killing them all. As I've stated before, no matter how wacked these people may seem, the further up the ladder of power they get, the less suicidal they seem to become. Their lives are immensely rich with servants everywhere, the best food, the best clothing, sex lives as kinky as they desire, people kissing their ass at every turn. When was the last time you saw a head of state truly do something suicidal or even risky? Hell, they don't even venture out in public without a nation of bodyguards. It just doesn't fit human nature. I truly doubt Hitler ever thought of his demise until those final moments in the bunker. Ego and a lust for life seem to go hand in hand.
When you think about it, the way to push a madman to suicide is to place their backs up against the wall with no way out. Only when the situation is hopeless does the absurd become rational. Of course exceptions can happen but you have to play foreign policy based on odds and history simply doesn't prove that the man will follow through on his threats.
