Evolution isn't random mutations.
Evolution of organisms is due to a biofeedback capability inherent in living cells. Organisms will literally mold themselves to fit an environment over the course of generations. I've developed a theory based on a moderized version of Lamarckian evolution. IMO, it answers many of the questions you raise.
