Bringing market forces into the medical field is the primary way to start bringing costs down. Reform of FDA rules would also be useful, as well as broadening the rules allowing you to get treatment from a wider range of practitioners.
The waivers are because "Obamacare" is rank lunacy, and survival for many companies requires being "allowed" not to commit the financial suicide the program demands.
