In my experience people get into this kind of trouble in one of two ways. First, they start out on some adventure into the mountains or onto the sea and the weather deteriorates. What was a delightful experience, easy, simple and safe all at once becomes a nightmare.
Second, if there is some equipment failure, including human bodies. A person could be sailing to Catalina, have a stroke and not be able to operate the radio or motor controls. Gone.
Or a person hiking into a small canyon in Arizona. Slips and breaks a leg. Easy to do on loose rock. Now, they don't have enough water to last for the time it takes to crawl out. Did have enough water for a simple 3 hour hike.
It's simple, really. And the person who IS ready for 98% of what happens looks foolish much of the time.

Very, very easily.
