modification: Porphyritic Andesite?
I see what you mean about the tabular laths. Plagioclase laths, possible amphiboles, +/- pyroxene, +/- potasium feldspar, then we should start leaning towards an overall intermediate composition - Andesite. Possibly with secondary alteration.
Another wedge shaped mineral:
Sphene (titanite). CaTiO(SiO4) more here
The wedge shape crystal is almost diagnostic under the polarizing microscope. The name sphene is derived from a greek word for wedge.
Our assumed mafic composition makes this less likely, unless you want to go the syenites and nephalines, or start thinking felsics. But maybe not. It's also common in alteration zones. Sphene would make sense in an Andesite over a basalt, IMHO.
