The point is who gets to decide.
Doctors? Patients? Political appointees? the NICE?
When the last nation succumbs to government health care, this kind of thing won't be an issue, since expensive drugs like Avastin won't exist any more. Look at that New York Times article, and you get some idea of what it takes to get an exotic drug to market.
