The question comes up in examples like that:

How much does it cost to give an entire population the examininations, drugs, and preventative care to help them avoid bypass surgery?

How many of those would have gotten the surgery without it? How many would have gotten the surgery anyway?

This math is more complicated than the simple assumption that preventative care will always save money.
