If the internal organizations of an enemy country are manufacturing and transmitting false information inside their organization, designed to fool their own superiors, it is asking a lot of our intelligence operations to see through that to the truth, don't you think?
If an Iraqi scientist writes a fake report with fake data sufficiently plausible to fool Saddam Hussein and his goons, it is highly unlikely that an intelligence operation observing from outside the system would realize his data was fake if Saddam and his people wouldn't.
Again, I am not saying this is how it has happened. I am speculating on a possibility.
