"ungrateful little shit" and "shit" don't mean the same thing- if they did we wouldn't bother adding the extra words. In informal use, you might call someone, a ungrateful little shit if you tried to do something helpful, but the person complains about something. Example, say you stop your car to pick someone up who is in the middle of a rainstorm, and guy starts ragging on you because you don't have a satellite radio:
"All you have is an am/fm radio?"
"Sorry, that's in my other car, you 'ungrateful little shit'. I got cd player with cowboy music, you want to listen to that?"
