m16 jamming problems

I thought the M16 jamming problem was attributed to a lower grade powder that was subsituted by DOD as a cost saving measure. I've read a lot of vietnam accounts and the jamming m16 is a common thread of nearly all of them. I wonder how many US military folks were killed byt that cost saving measure.
