The original M-16 didn't have a bolt assist to help it seat. The M16A1 did have that along with a chromed receiver. I'm not denying that the ammo contributed to the problems, but there were other issues as well. It helps to remember that the M-16 was originally an Air Force weapon. The Army adapted it later.
