Look at the Veterans' Administration
In the late '90s it became excellent at bargaining with drug companies and delivering health care to veterans.
Instead of leaving it to private organizations who will only insure the healthy, why not look to the VA as a model of large-pool health care that spreads the risk more evenly?
I have two kids now whose health insurance consists of being in their 20s. This does not make me feel secure for their future.
