First of all, the dialysis thing is probably a myth. Bin Laden has denied it himself, and there's no real evidence. Second, he doesn't exactly show up in studios for Arab TV interviews. He's somewhere in Pakistan, near the northwest borders, in the mountains. The government of Pakistan doesn't control these areas very well. Tapes, of which the last video version appeared almost two years ago, are transported by various couriers.
Third, if you did spot a 6'5" person--and I'm sure he's the only person in Pakistan that tall--would you bomb the village he's in? Remember, the police don't go there.
