Is mandatory insurance really required to make it work?

In Canada, being part of a govt-run insurance plan was optional...at least that was the case when I left. If you preferred a private plan, or no coverage at all, that was your choice.

Basically, you had to enroll annually (a very simple opt-in form), and if you failed to do so you were out, at least until the next chance to opt-in, a minimum of a year in the future.

My understanding is that very few people opted out, naturally, and those who did were typically wealthy. For them, opting out was as much symbolic as practical.

Anyway, the point is that you could opt out...be uninsured if that was your choice...but could not get back into a govt plan just any old time you felt like it. Few chose to opt out, and it all seemed to work under those conditions.
